• The $25.00 registration fee must be included with your completed registration form. Registration fee is NON-REFUNDABLE. ***Note if you are registering as a DUET ~ each participant MUST fill out a registration form, and fee is $25.00 per person.
• Registration should be dropped off or postmarked by Friday, June 10th, 2011. The Community Foundation of Carroll County, Inc. is closed on Saturday and Sundays.
• Any missing documentation will result in immediate disqualification.
• All Judges Decisions are final.
• You will be notified by mail of your future participation.

• Completed Registration Form with necessary signatures
• $25 check for Registration Fee
